fix(storage): make block map access thread-safe and look up block metas by block id - #3533
NewSegmentSnapshot::block_map() built block_map_ without a lock while callers iterated the
reference it returned.
SegmentMeta::AddBlockID1 did back() + 1 / push_back outside any
lock — two appends to the same segment could be handed the same block_id, and the push_back
could reallocate the vector under a reader.
BlockIndex::GetBlockMeta also treated block_id as an index into block_map(), but block_map() is built from GetTableSegmentBlocks, which sorts and filters ids to those
visible to the current transaction. Ids are therefore sparse, and any gap in the middle made the
scan read a different block's rows silently.
Changes
NewSegmentSnapshot::block_map(): build block_map_ under a new block_map_mtx_.
SegmentMeta::AddBlockID1: hold mtx_ across the whole read-modify-write; inlined and removed GetBlockIDs1(TxnTimeStamp), which split that critical section by returning a raw pointer.
GetBlockMeta: binary search on block_id instead of positional indexing.
MatchTensor rerank: use GetBlockMeta instead of .at(block_id) (returned no status on a miss).
TableScan / MatchSparseScan / MatchTensor rerank: UnrecoverableError on a null BlockMeta
instead of dereferencing it.
PhysicalTableScan: reset range_state when a block is skipped by FastRoughFilter; it was reused
for the next block with stale visible ranges.

Segment block-ID access and snapshot block-map access now use mutexes. Metadata lookup searches by block ID. Table, sparse, and tensor scans raise an unrecoverable error when block metadata is missing. Table scans also clear visible-range state when the fast rough filter skips a block.
GetBlockIDs1 no longer takes a timestamp. AddBlockID1 loads block IDs directly when the cache is empty and updates the cached IDs under a mutex.

NewSegmentSnapshot adds a mutex for block-map access. block_map() uses the mutex, NewInit() uses try_emplace, and GetBlockMeta() searches by block ID and returns null when no exact visible match exists.

Table, sparse, and tensor scans raise an unrecoverable error when block metadata is missing. The table scan also clears visible-range state when the fast rough filter skips a block.
The changes improve synchronization and sparse block-ID lookup. No merge-blocking regression was established; the existing unlocked block-ID reader exposure remains a separate follow-up.

A rabbit checks each block in turn, Finds missing metadata, raises alarm. The block map waits beneath its lock, IDs line up by search, not slot. A skipped range is cleared away, Then scans continue on their way.

Major · Stability & Availability · Quick win
Move existing block-ID iterations to the locked-copy accessor.
When AddBlockID1() appends during GetSegmentInfo() or MapMetaToSnapShotInfo(), those methods still iterate the vector returned by GetBlockIDs1(). The append can reallocate the vector and invalidate the iteration. GetBlockIDsCopy() fixes this only for callers that use it. Update the existing iterating consumers, including the secondary-index path, to use the copy. (raw.githubusercontent.com)
